No later than Thursday, July 28, PRIOR TO REGISTRATION on Friday, July 29, parents and players are REQUIRED to watch the ASAA "Play for Keeps" video and BOTH are REQUIRED to sign ASAA's acknowledgement form Bring this completed form with you to registration. If you have questions about this policy, please review the ASAA website and/or go direct to the ASAA policy There are no exceptions to this rule. Registration will not be allowed unless both players and parents have watched the video and signed the acknowledgment form.
NEW FOR THE 2011 SEASON!!! The Anchorage School District now requires EVERY student athlete to complete Concussion ImPACT Baseline Assessment in the form of video questionnaire by each athlete. THIS CANNOT BE DONE AT HOME, IT MUST BE DONE ON AN ASD COMPUTER!!! Coach Lewis plans to have Georgia Military Camp player attendees complete this testing sometime during camp next week. We will keep you updated as we are informed. In the meantime, parents, please print the parent consent form at Baseline ImPACT Concussion Assessment Parent Permission Form and be sure your player has that signed form at camp and at registration. This is a one-time assessment for every student athlete, so it will not be an annual requirement and your player will not have to repeat it for every sport.
Friday, July 29 at South, 8-11am and 12-3pm: Football Program Registration Day for both ASD and South Football. DO NOT MISS THIS REGISTRATION DAY. What you need to know about registration:
1) You will start at the South Activities Office. There you must provide:
a) Completed ASAA parent and student acknowledgment form that BOTH you AND your player have watched the Play
for Keeps video "Play for Keeps Video" and ASAA's acknowledgement form
b) Completed ASAA health exam form
c) Completed ASD Participation form
d) Completed Sports Fee Remittance/Waiver
e) The $185.00 player fee (or fill out the fee waiver portion on the form in paragraph (c)
f) $5.00 fee AND SIGNED Baseline ImPACT Concussion Assessment Parent Permission Form for Concussion Assessment to be taken by the player on an ASD computer either prior to or on the day of registration (assessment is by video questionnaire and takes 20 minutes!)
2) After you get through the Activities office, you will go through the final registration process with the South Football Program. The Starter Pack/Registration fees are $225.00 for Grades 9-12. Please note this cost is in addition to the ASD fee. It is separate. This is because South Football has no control over the ASD player fee, and we do not receive any proceeds from it. We are sorry, but we cannot change this. Parents and players can participate in pre-season fundraising to cover these costs prior to registration.
